Mary’s Meadow is a skilled nursing and post-acute rehabilitation program located in Holyoke, MA. It consists of 4 small houses with 10 private rooms and bath and a living and dining area in each house. Each has its own entrance and each opens into a central chapel which allows staff to move from one house to the other internally. Each house has its own staff, a domestic kitchen where the meals are prepared for the 10 residents who live in that house It is developed around a concept of making the residents feel “at home”. It is their home and they are encouraged to participate in activities and engage with staff in their care and all the activities of daily living. It is a “social” model not a “medical” model of care.
The need to keep our residents safe and
free from Covid-19 as with any other Skilled Nursing Facility has presented many challenges. Explaining the need for masks to wonderful people who do not remember, to those whose comprehension is “not what it used to be” and the need for isolation when a staff member has tested positive and we have to go into “lock- down” has presented major challenges.
This is especially difficult in the Small House Concept where the major goal is so- cialization and bringing people together in a “family like setting”, the basic framework of all our activities of care.
Our residents are so used to being able to move around in “our home” and are encour- aged to spend time in the living-dining area and not isolate in their rooms; socialized dining even for those who are not as alert and who need assistance with eating is an aspect that increases their belonging when they hear the conversations around them.
Little things like being together when someone is reading the news of the day from the local newspaper, group exercises, even activities like Bingo fill the day and keep our
resident “family” socialized and involved. Many of our residents do not understand
the need for staff to wear gowns, masks and face shields. They look forward to a hug from the staff members and now there is none of that.
The pandemic has changed so much. Our residents are confined to their rooms much of the time and it has placed a greater burden on our already overtaxed staff who before they enter to engage with the resident, must gown and wear protective gear. They spend time individually with the resident in their rooms to assure that we do not lose the value of keeping each one socially connected.
Let us remember those who work at Mary’s Meadow and in all nursing facilities and thank them for having made the adjustment and con- tinue to deliver quality care under very different and much more difficult circumstances. It is not easy. We know that and we thank each one for adjusting, for going the extra mile and above all for keeping our residents safe from Covid-19.
