Fallon Expands Senior Program

WORCESTER — Fallon Community Health Plan (FCHP), a nationally recognized, not-for-profit health care services organization, announced the expansion of its Medicare Advantage special-needs plan, NaviCare HMO SNP and NaviCare SCO, a Senior Care Options program.

In addition to still being offered in Worcester County, NaviCare will also be available (effective Jan. 1, 2012) to eligible seniors in Hampden County and portions of Franklin, Hampshire, Middlesex, and Norfolk counties.

NaviCare is a unique product for seniors that combines in one package all Medicare and MassHealth Standard benefits, services, and items, as well as needed community-based home and personal-care services such as adult day health, home-delivered meals, or help with bathing and dressing needs. NaviCare is available to those residing in the service area that are age 65 and over who are enrolled in MassHealth Standard.

For those who qualify, NaviCare is available at a $0 premium with $0 co-payments. Members receive high-quality care through the program’s ability to combine health and social services. All health care is coordinated by a team of doctors, specialists, nurses, and social workers who work with each member to develop a plan of care specific to them.

Unique to FCHP’s NaviCare program is the role of the ‘navigator,’ who helps manage the members’ health care needs, including some of the tasks that may be typically performed by a caregiver. For more information about NaviCare, visitwww.navicare.org.
