Holyoke Medical Center Requests Community Response to Health Survey
HOLYOKE — Holyoke Medical Center is conducting a community health-needs assessment to identify and address the most pressing public-health needs in the Pioneer Valley. Community members are encouraged to participate in this process by taking the Community Health Survey. The link to the survey, in both English and Spanish, can be found at www.holyokehealth.com/chna.
The survey will take 20 minutes or less to complete and will be available through April 20. Survey respondents will be identifiable only by ZIP code of residence, and all individual responses will be kept confidential.
All survey respondents will have the option to enter a drawing for one of several $100 gift cards. Personal contact information entered for drawing registration will be kept confidential and used solely for the purpose of this drawing and not for any marketing purposes.
The purpose of the survey is to gather as much information from area residents on what they feel are the most important health challenges facing the community. Holyoke Medical Center will share the results of this survey in an online comprehensive report that can be accessed on the hospital’s website. This report will be used to identify health trends and challenges.
