Noble Hospital to Offer Free ‘Compass to Care’ Program
WESTFIELD — Noble Hospital will host a free informational program called “Compass to Care — Navigating the Transition from Hospital to Home: How to Help Yourself or Your Elders.”
This program is open to the public and will be held on Wednesday, April 9 from 5:30 to 7:30 p.m. in Noble’s Conference Room A. “Compass to Care” is designed to give an overview of services available to assist patients and their families in moving from hospitalization to home or a nursing home, assisted living, or another facility.
Speakers and topics will include:
• Shannon Fournier, Noble Hospital: “Your Hospital Stay”;
• Jennifer Crain, Noble VNA: “Home Care”;
• Chelsea Roberts, Genesis Health Care: “Skilled Nursing Facilities and Long Term Care”;
• Sondra Jones, the Arbors: “What Is Assisted Living?”;
• Beth Cardillo, Armbrook Village: “Assisted Living for Memory Loss/Dementia”;
• Tina Gorman, Westfield Council on Aging/Senior Center: “Community Services”; and
• Julie Chandler, Highland Valley Elder Services: “Community Services.”
