Baystate Noble Hospital Announces Destruction of Medical Records

WESTFIELD — Baystate Noble Hospital will destroy medical records older than 20 years on Saturday, Jan. 2, 2016.

The Massachusetts state regulation for the retention of medical records is 20 years. If the last time you were a patient at Baystate Noble Hospital (previously known as Noble Hospital) was before Dec. 31, 1995, your record is scheduled to be destroyed on Jan. 2. If you were a patient at Baystate Noble Hospital after Jan. 1, 1996, your record will be kept in its entirety. Department records affected include inpatient, outpatient, observation, ancillary, same-day surgery, medical day care, and clinic.

A request in writing may be sent to HIM Department, Baystate Noble Hospital, P.O. Box 1634, Westfield, MA 01086-1634, and must include the date of your request, your name at the time of the visit, your current name (if different), your date of birth, and your signature. Upon receipt of this request, a three-year abstract (face sheet, discharge summary, history, physical, operative report, and emergency-room paperwork) of your records will be copied and sent to you at no charge. If more copies or years are needed, a fee of 74 cents per page will be charged.
