Visiting Angels West Springfield Partners with PocketRN

HOLYOKE — Visiting Angels of West Springfield announce a new partnership with PocketRN, a Medicare-approved Guiding an Improved Dementia Experience (GUIDE) dementia care provider. PocketRN delivers GUIDE care virtually with expert nurses who will help seniors and their loved ones through every step of their care journey.

Seniors with Medicare living with dementia may qualify for Medicare’s groundbreaking new GUIDE program at no cost. To qualify, clients must have dementia, be enrolled in Medicare Parts A & B (Original Medicare), and not be on hospice or living in a skilled nursing facility.

In addition, by participating in the program, seniors may be eligible for an annual 72 hours of paid home care hours. To be eligible for this additional benefit, clients must have moderate to severe dementia and have a family or unpaid caregiver. They do not need to live with the individual with dementia.

This program comes highly recommended by the Visiting Angels team, as dementia-trained remote nurses work closely with seniors and their caregivers to monitor and deliver care over video and/or phone call. Clients and their loved ones will experience greater peace of mind when it comes to navigating changes and getting quality, personalized care with convenience.

GUIDE care from PocketRN includes a ‘nurse for life’ who performs a detailed, dementia-focused initial assessment over video chat and follows up with regular check-ins, which allow nurses to monitor health, provide key preventive care, and catch issues early; on-demand care when questions come up; a way for families to stay informed; care coordination with existing doctors, specialists, medical equipment suppliers, and Visiting Angels; medication management; family support, one-on-one coaching, and education; management of other chronic conditions like diabetes, cancer, heart disease, etc.; and more.
